Decide what gets flagged and how urgently: fluid below target, mood dropping from someone's usual, a fall or concern. Each raises an amber or red alert, at thresholds your staff set.
Your staff decide what phrases mean. "A cup of tea" is 200 ml of fluid; "very happy" can be a 5 for mood. Alerts then fire on a real change, not a word.
